Looks to be a precipitate, meaning a dissolved solid that has fallen out of suspension in the liquid. I'd bet five dollars a good hot water bath and a vigorous shake will get it back to normal.
Diacetyl is a liquid and though it may be present in this flavor, that is most certainly not what is causing the crystals.

Thank you for your enquiry with Wizard Labs. Malted Milk is one of those flavors that will crystalize quickly. The flavor is not bad or spoiled, to let you know. In order to get rid of these crystals, try shaking the flavor vial. If this does not alleviate the problem try microwaving the vial in short bursts of no more than 3 seconds at a time. Much more than this will superheat the glass, please be careful. After heating, shake the vial again and it should mix into 1 smooth solution again. I've never seen that in my Bavarian cream. It's also usually a lot darker IIRC. Is this the new DX version?
i have tfa Bavarian cream from last year that i have in the fridge and it did the same thing, but the same flavor that i got from tfa a week ago and that have been in the fridge since then, did not crystallize!
