I created this calculator using Microsoft Excel 2013. For anyone without Excel, I have also tested it on OpenOffice Calc and it works just as well.
Calculator features basic ejuice calculation, PG/VG ratio calculation, and calculation for multiple flavors. I have checked and rechecked the math and it has proven to give accurate measurements.
User inserted values are marked in a black border while calculated measurements are highlighted yellow. There are notes next to cells to help better understand how they work.

Edit: I thought of a new feature that for each of the flavoring name cell there is a drop down list with all TFA flavors. If I missed any, let me know. I also plan on doing the same for Capella and FlavorArt sometime soon.

You're welcome, I made this in mind for everyone to use. My idea is that using readily available software, we can easily make recipe books for ourselves and also to share with others.
I have an almost completely opposite take on the same general idea - readily available software, easy to share, recipes useable with or without the software. It's vaper-tools. There are some examples from the Juice Talk show here like the evidently tasty Fuzzy Custard. It has output plugins for a couple of text formats and a nice-ish HTML page, and it can also read/write eJuice Me Up files.
There's also a quickmix frontend for off the cuff usage:

Sadly there's a new juice calculator / recipe website / whatnot of some sort almost every week and none seem to gain a lot of traction. :(

I know drops are frowned upon... but for quick mixes and experiments, they are sometimes handy. Could you add a column which does a ml/drops conversion, and have one drops/ml calibration entry (example: 30 drops/ml)?
